Nurse-Transplant Coordinator - Heart/Lung

The Heart/Lung Transplant RN Coordinator works under the direction of the attending surgeon, cardiologist and pulmonologist, and within a multidisciplinary team and is responsible for the coordination of care for patients in outpatient settings and inpatient education. The patient population includes patients being referred and evaluated for transplant, those currently listed for transplant, post-transplant patients, and those who are being considered for mechanical circulatory support/ventricular assist devices (VAD). Collaborates closely with the VAD NP to ensure overall coordination of care for VAD patients. Serves as a liaison between the Transplant Program and the many other disciplines that assist in caring for transplant and VAD patients. Interfaces with physicians, fellows, residents, nurse practitioners, pharmacist, service program support assistant and clerks Covers a broad range of activities for patients in the referral, evaluation, peri-operative, and post-operative phases of transplantation and/or VAD placement. This may include tracking results of scheduled tests and imaging, obtaining outside records, ordering imaging and laboratory testing, providing patient and family education, triage, discharge planning and post hospitalization follow up. Assists the team in providing emotional and educational support to veterans, their families, and caregivers as needed. Responsible for managing and coordinating comprehensive nursing care for patients whose needs range from uncomplicated to complex and rapidly changing. Completes focused assessments of outpatients. Orders laboratory tests, radiographic tests, prosthetics consults to include VAD equipment, and adjusts medications following written protocol. Provides documentation to meet transplant regulatory requirements. Participates in and documents discharge planning. He/she also evaluates patients in the Nursing Transplant Clinic following clinic protocols and consults with physician for situations not covered by these protocols. Communicates frequently with referring providers to coordinate all aspects of transplant and VAD care. The RN serves as a clinical resource for other interdisciplinary team members and addresses system issues as they arise. Completes necessary documentation to maintain compliance with VA National Surgery Office (NSO) regulations and the United Network for Organ Sharing (UNOS) requirements, including presentation of patients at the Patient Selection Committee meetings. Provides on-call services during non-administrative hours for patients and medical/nursing staff at the hospital and home VA medical centers who need guidance concerning the triage of transplant and VAD patients, and for arranging and organizing transplant operations. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
