Option to work 4 or 5 daysThis is a full-time physician role caring for Medicare and Medicaid geriatric patients in the home setting.
Our goal is to see patients with realistic time, to provide the right level of care to make the clinical impacts outlined below:
The Physician Team Leader (Pod Leader) acts as a resource/coach impacting clinical performance of the team.
The leader will collaborate with advance practice providers and build a collaborative team.
Success in Value Based Care changes the demands placed on primary care.
